Sue Gray said 16 events fell within her investigation's remit, of which 12 are now being investigated by the Met police.
The senior civil servant said some events "should not have been allowed to take place".
Boris Johnson has responded saying he was making changes to how Downing Street and the Cabinet Office is run, so they can get on with the job of government.
