2:16Now PlayingColumbia University Medical Center's Dr. Sanja Jelic says proning, or lying on one's tummy, could increase oxygenation, which could aid coronavirus victims.
Sanja is a physician and associate professor of medicine at Columbia University in New York. She is also the also the co-author on a paper about the use of the proning position in awake, non-intubated patients.
Intubated patients are routinely turned on their tummies. But as Sanja explains, necessity drove her to try the technique on non-intubated patients.
“On April 6th in New York from a personal experience, I think that was the worst day hospital wise. We were really, really very busy. At some point we had 68 patients. Many of these patients worsen and then they need intubation and transferred to the intensive care units. And we have converted many hospital spaces into pop up ICU intensive care units. However, despite all our efforts, we were stuck with several patients who needed to be intubated. However, we just couldn't intubate all of them at once. So we had to buy time. And I made the clinical decision to put these patients in a prone position.”
She continues, “We, of course, applied for the approval to track those patients. And once we received the approval, we just looked at our outcomes and reported it in a study. We were hoping that this would be helpful to other health care providers treating Covid patients.”
Sanja says the prone position could help all parts of the lungs get ventilated.
“There's more uniform ventilation and perfusion. So there's more matching. These upper portions of the lungs get some blood and they also get some air. So the body is able to extract more oxygen and get rid of some more carbon dioxide more effectively. And we see that increase oxygenation measured by pulse oximeter on a finger. So we think that's what's at play here.”
