The tragedy occurred during the FA Cup semi-final against Sheffield Wednesday, at Hillsborough Stadium on 15 April 1989.
The report by the IOPC found that 12 police officers would have faced gross misconduct charges, including for "concerted efforts" to wrongly blame fans for the disaster.
But because so much time has passed, nobody will face disciplinary action and families say justice is still being denied.
