Kay had a long history of mental health problems and in just 19 days in prison, incidents of self harm were officially recorded 11 times but never properly acted on.
Oceana Jane says Kay should never have been in prison in the first place.
The Ministry of Justice told us: "The government has set up a Women's Justice Board to advise on reducing the number of women in jail and are changing the law so more people will serve their sentences in the community."
