1. What are the worst possible outcomes if I pursue [ISSUE]?
2. What can I do to prevent these outcomes from happening?
3. If all these outcomes were to occur, how would I recover?
If you're really invested in beating fear of failure, you'll run one of these worksheets for yourself. The simplest way to overcome fear of failure is to exorcise the demon you're making it to be in your mind. Expose the real consequences and make a plan for how you'd bounce back if you had to deal with them.
