Dementia is a horrible and increasingly common problem, particularly as our population ages. Poor sleep in middle age has been associated with later development of depression. Whether insomnia causes dementia, whether it is just a very early sign, decades before other dementia symptoms appear (reverse causation) or whether the association exists because some other factor makes brains susceptible to both insomnia and dementia, remains unclear.

Several large studies have failed to resolve the debate on a related issue, of whether the use of sleep agents such as Ambien (zolpidem) increase the risk of developing dementia.

A recent study by Maiken Nedergaard, who discovered the glymphatic system, uncovered how this system (the dishwasher of the brain) expels toxic debris. Slow cycling of the firing of norepinephrine cells, about once ever 50 seconds, contracts blood vessels through the brain, increasing the space between cells, and flushing out toxic proteins.

Nedergaards group also showed that, at least in mice, Ambien significantly impaired this glymphatic pumping system.

But insomnia itself almost certainly interferes with a normally functioning glymphatic system, so it is not clear who might benefit from long term Ambien use, and who should avoid it. Most research overwhelming supports the use of sleep hygiene and CBTi (cognitive behavioral therapy for insomnia) approaches before trying medications for sleep problems.
