Spending on both programs has exploded, and today they account for more than one third of all health spending. Medicare is generating trillions of dollars in long-term debt. Medicaid is a major challenge for state budgets. In both programs, doctors must wrestle with bureaucracy and red tape and low reimbursements, and patients are threatened with reduced access to care.
