Citing multiple sources familiar with the decision-making process, CNN reported that warnings about old intelligence were present in Pentagon systems used to prepare target lists. The process required senior officers to approve the inclusion of such sites, but commanders moved ahead with the strikes amid pressure to quickly finalise targets at the beginning of the conflict.
The February 28 strike on the Shajareh Tayyiba school in Minab, southern Iran, took place on the first day of US military operations against Iran. Iranian state media reported that at least 168 children and 14 teachers were killed in the attack. US military officials later determined that outdated intelligence contributed to the strike. One source familiar with the matter said officials “knew within days of the strike how the mistake happened.”
