Narcissists are fully aware of their behaviors, but not of their motivations.

But they have different valence, impaired reality testing, and alloplastic defenses. They often misjudge whether their actions are morally right or wrong.

Still, they should be held accountable. They are not insane in the legal sense (though they probably are from the psychiatric POV).
