Why Paraffin Was First Used

The very first reported use of paraffin injection was by Austrian surgeon Robert Gersuny in 1899—not for cosmetic reasons, but to simulate testicles in a man who had lost his in surgery, to help him pass a military physical exam

Shortly after, the technique spread to breast augmentation—primarily for aesthetic purposes. Early 20th-century women, particularly in Europe and Japan, sought fuller breasts for reasons such as:

Cultural ideals of femininity

Postpartum volume loss

Economic incentives, especially among sex workers and entertainers (e.g., Japanese women competing for the attention of American servicemen during WWII)

What Paraffin Is Chemically

Paraffin is a mixture of saturated hydrocarbons (alkanes), typically derived from petroleum.

It is solid at room temperature but melts around 47–65 °C.

It was liquefied for injection and then hardened inside the body.

Why It Was Abandoned

Despite initially promising cosmetic results, paraffin injection quickly became notorious for severe complications:

Chronic inflammation

Formation of hard granulomas ("paraffinomas")

Infection, ulceration, and tissue necrosis

Migration to other tissues

Permanent disfigurement

By the 1920s, the use of paraffin for breast enlargement was largely abandoned by the mainstream medical community, although underground use persisted in various forms for decades.

In sum, paraffin injections were driven by a mix of medical curiosity, aesthetic demand, and early commercial plastic surgery marketing—long before implants were developed.
