Independent midwife Donna Ockenden said action plans to improve were "put on the shelf" and the stories which her report team uncovered were "harrowing and distressing."
More than 200 baby deaths could have been avoided and seven mothers died needlessly in what is being called the UK's biggest ever maternity scandal.
