A family have told Sky News they want “someone to take accountability” after their father was stabbed to death by a former secure psychiatric unit patient.

Retired butcher Lewis Stone was killed in the Welsh town of Borth while he was walking his dog.

He was attacked by David Fleet, who had been released from a secure psychiatric unit 10 days earlier.

In her first TV interview, Lewis Stone’s step-daughter, Vicki Lindsay, said the family were calling for an internal NHS trust report to be made public in an attempt to stop a similar attack happening again.

David Fleet's family declined an opportunity to speak to Sky News for this report.
