The Prisons and Probation Ombudsman has issued a report into how physical and sexual violence was allowed to continue at the Medomsley Detention Centre from 1961 to 1987.
Sky's correspondent Greg Milam looks at how the abuse at Medomsley went “unchallenged” for the entire 26 years of its operation.
