NHS trusts across England are being investigated over failures in maternity services, with experts warning that women and babies are not receiving the "safe, compassionate care they deserve".

Issues dating back more than 15 years are being investigated across 12 trusts - and the review's chair says the findings will lead to nationwide improvements.

Sky News has spoken to three women who suffered traumatic births in NHS England hospitals.

They say they were failed – and made to feel voiceless.
