Baroness Amos published her initial reflections following meetings with families and seven of the 12 NHS trusts under the microscope.
She said what she had found so far was "much worse" than anticipated.
The report is the first step towards a set of national recommendations that will be handed down next year.
But the victims of maternity failings say they're "disappointed" with the findings and fear the body will have "no teeth" to make changes.
