Home Secretary has called Sir Adrian Fulford's report "heartbreaking", while acknowledging systematic failures in the run-up to Axel Rudakubana's attack.
The report showed a "systematic failure of the state to prevent a vile and sickening individual perpetrating this atrocity", Shabana Mahmood said in response to Fulford's findings.
Mahmood also paid tribute to the victims and their families, adding the "senseless killing of three young girls" had left a "lasting mark on our national conscience".
