9:10Now PlayingDonna Ockenden, who led the largest maternity review in NHS history, says the government's response to the Nottingham maternity scandal is not moving fast enough.
In an interview with Sky's Sarah-Jane Mee, Ockenden argues that her findings should be treated as "immediate and essential actions" rather than recommendations, warning that waiting months for a maternity action plan risks further harm to mothers and babies.
The review, which examined the experiences of 2,511 families and heard from 838 staff members, uncovered systemic failures, cover-ups and serious concerns about patient safety at Nottingham University Hospitals NHS Trust.
