The NTSB has released its preliminary report on United Airlines Flight 169, the Boeing 767-400 that struck a light pole while landing at Newark Liberty International Airport on May 3, 2026. The report reveals that the aircraft was only about 19 feet above the New Jersey Turnpike and had been below the normal glide path before impacting the light pole. Investigators also found that the aircraft did not actually strike the bread truck directly. Instead, debris from the light pole damaged the truck and injured the driver.

In this episode, retired airline captain Captain Steeeve breaks down the approach, the crew's decision making, the first officer's "slow and low" callouts, stabilized approach criteria, and the question every pilot is asking:

Why wasn't a go-around initiated?

Join Captain Steeeve as he analyzes the preliminary findings and explains the lessons every pilot can learn from this incident.
