3:50Now PlayingAxel Rudakubana would not have carried out the Southport attack if his parents had reported what they knew about their son to the authorities, a public inquiry has found.
Chairman Sir Adrian Fulford has said the murders of Alice da Silva Aguiar, nine, Bebe King, six, and Elsie Dot Stancombe, seven, at a Taylor Swift-themed dance workshop on 29 July 2024 "could and should have been prevented".
Sir Adrian found that Rudakubana's parents "ultimately failed" to report the risk their son posed - and he also identified several major areas of systematic failure within the police, security, health and social care services.
