Nursing homes are required to keep detailed records of the care they provide to individual patients. Those records are supposed to be, not only in the nursing notes for the each and individual patient, but also they're obligated to provide any medication logs and also any outside of the normal events that occur have to be cataloged and those have to kept inside the patient chart.
