Liz Bethard, a leader at Glenn Medical Center, watched the only hospital in Glenn County lose its federal critical access designation after CMS measured it at 32 miles from the nearest hospital instead of 35 — using Google Maps. The result is a 40% funding cut and an emergency room that closed on September 30.

When the hospital flew its administrators to Washington for a scheduled meeting to fight back, Dr. Oz's agency canceled it and never rescheduled.

Glenn County is a rural, working-class community in California that voted heavily for Donald Trump. The critical access hospital designation exists specifically to keep rural hospitals alive — these facilities serve smaller, lower-income patient populations that cannot sustain a hospital on standard Medicare reimbursement rates alone. When CMS stripped Glenn Medical Center of that designation, it wasn't based on a federal road survey or a site visit. It was based on a Google Maps measurement that the hospital's own administrators disputed, explaining that 32 miles reflects a route ambulances do not actually use. That explanation was submitted. It didn't matter.

The human cost Liz Bethard described is direct and specific. A heart attack or stroke victim in Glenn County will now face a 30 to 40-minute drive to emergency care, in a county where ambulance availability is already strained. Bethard said publicly that her own father, who has a history of heart attacks, could die because of this decision. This is what happens when ER doors close.

The broader context makes this impossible to wave away as an isolated bureaucratic error. Over 700 rural hospitals across the United States are currently at risk of closing. Rural healthcare access was already at crisis levels before Trump's second term began. CMS reviewed Glenn Medical, stripped its designation, canceled the meeting where the hospital came to fight for its survival, and moved on. Rep. Doug LaMalfa, the Republican who represented this district before he passed away, told the hospital not to close too fast while he works on legislation, which did not arrive quickly enough to save them.

This is what America First looks like from inside a rural county that gave Trump their vote.
