This report is been submitted to Chief Minister Jagan Mohan Reddy. It lists various causes of the accident including poor safety protocol measures, faulty cooling system, insufficient knowledge among staff, breakdown of emergency measures response, flouting of lockdown norms, negligence of officials, and slackness of management, inadequate risk assessment response and much more. The alarm systems too were not used by the management and employees in the factor. The Chief Minister has ensured to undertake strict action against the factory authorities.
