But after 7 weeks and 3 different healthcare facilities, it was finally looking like she would be able to go home. However, it was soon after they did one more routine exam, when her discharge plans were put on hold. Jennifer's condition began to get worse. Soon after, there'd be a major communication error during the medical handoffs of Jennifer. And after a misdiagnosis and multiple transfers between different hospital units, her care team(s) would continue missing the signs that something was wrong, effectively giving her treatment that only exacerbated her already worsening condition.
